Prévalence de l’angine streptococcique chez l’enfant de 12 à 35 mois aux urgences pédiatriques : résultats intermédiaires de l’étude Pasteur

Abstract : Introduction. Streptococcal pharyngitis is acute inflammation of the oropharynx caused by group A streptococcus (GAS). In France, it is considered rare in children under 3 years of age and it is neither screened nor treated. The aim of this study was to evaluate the prevalence and factors associated with streptococcal sore throat in children aged from 12 to 35 months old. Methods. We carried out a single center interventional prospective clinical study in the pediatric emergency department at Annecy-Genevois Hospital. Patients aged from 12 to 35 months old with a clinical picture of acute pharyngitis were included. The collected data came from the clinical examination, rapid antigen detection test (RADT) and from legal guardian’s interviews. Results. Forty six of the 1341 patients who accessed pediatric emergencies fulfilled our inclusion criteria. The sex ratio was M/F 0.6 and the mean age was 24.12 (SD 11.6) months old. The prevalence of GAS pharyngitis was 26.1% in the study. Neither anamnestic nor clinical criteria were significantly associated with a positive RDT result. Prevalence of SBHA pharyngitis was not higher in children in collective child-minding. Conclusion. The prevalence of GAS pharyngitis was 26,1% in children between 12 and 35 months old. The bacterial origin of pharyngitis does not seem to be determined by simple clinical and anamnestic criteria. RADT would allow a reliable diagnosis and could thus lead to a better antibiotics prescription in young patients. Further studies are needed to confirm this hypothesis.
