Les patients hypertendus possédant et ne possédant pas leur propre autotensiomètre ont-ils des comportements différents ? Enquête chez le généraliste en Ille-et-Vilaine

Abstract : Background : In the international literature, the superiority of home blood pressure (BP) measurements is accepted. In France, health authorities prives BP monitors to General Practictionners (GP) to encourage home BP self-measurement (HBPM) based on good practice recommendations from the Haute Autorité de Santé (HAS). There is no official recommendation for the hypertensive patients to have a BP monitor at home. Objective : The aime of this descriptive study was to analyze and compare the knowledge and behaviour of hypertensive patients according to whether or not they have a BP monitor in order to allow the GP to improve his guidance and care for these patients. Method : Self-administrated questionnaires were given to hypertensive patients of Ille-et-Vilaine having and not having a BP monitor between January and April 2016. Results : 170 patients with and 130 without BP monitor answered. 37.0% of patients owning a BP monitor know their BP target against 24.2% of patients without (p=0.0088). Patients without BP monitor respect the rules of HBPM good practice better than patients owning one. Among patients owning a BP monitor, those who received explanations on its use respect the rules of HBPM godd practice betterr. Patients owning a BP monitor report a better understanding of the importance of managing high BP (p=0.0172) and declare to be more anxious about their BP results (p=0.0221). Conclusion : there are some moderate differences in behaviour among hypertensive patients having and not having a BP monitor, but this study does not highlight criterias that could motivate the GP to advise the hypertensive patients against the purchase of a BP monitor. In all cases, it is essential to educate the patient about his BP monitor according to the rules of HBPM good practice.
