Abiratérone et cancer de la prostate métastatique réfractaire à la castration : efficacité et tolérance en population non sélectionnée

Abstract : Background: abiraterone acetate, an androgen biosynthesis inhibitor, is now a standard of care for men with metastatic, castration-sensitive and castration-resistant prostate cancer (mCRPC). Data exploring real-world toxicity and outcomes are lacking. Methods: retrospective study on unselected patients with mCRPC under therapy with abiraterone. Results: 93 patients were included in the study. Among them 58 (64%) had previously been treated with chemotherapy. Over a median follow up period of 19.2 months, median overall and radiographic progression-free survival (PFS) were 18 months (95% confidence interval [CI], 14.7 to 25.4) and 7.3 months (95% CI, 5.7 to 10.3) respectively. Safety and overall survival assessments in patients with and without prior chemotherapy were similar to that previously reported in phase3 randomized trials. PFS was also analyzed for adverse events of special interest. Median PFS were 7.5 months for hypokaliemia and hypertension, and 5.3 months for liver-function test abnormalities (not reached for cardiac disorder). Conclusion: our findings provide further evidence for the survival benefits of abiraterone with a low frequency of additional adverse events among unselected patients. Adverse events may be prevented through long-term clinical and biological monitoring.
