Impact d'une hospitalisation sur la prescription médicamenteuse du sujet âgé : mesure de la réévaluation des traitements potentiellement inappropriés selon la version V2 des critères STOPP and START

Abstract : More than 10% of hospitalizations in the elderly are attributed to serious adverse drug events (ADE). Many screening tools for detecting potential inappropriate prescribing (PIP) have been developed in order to guide physicians in the process of optimal prescribing. This study has been conducted in our teaching hospital after selected care services with pharmaceutical presence. A comparison by the new STOPP and START criteria was established between the prescriptions of older patients on admission and on discharge. Of the 166 patients included, the prevalence of the PIP on admission was 56% . Although the average number of medicines by patients increased by 18.6%, discharge prevalence of PIP was reduced (37%). Over half of the PIP have been discontinued during hospitalization (56%). The most high-risk drugs detected in our study were loop diuretics, benzodiazepines and opioid analgesics. Communication among health care providers is fundamental to promote the maintenance or the revaluation of introduced treatment, depending on the evolution of clinical events. A multidisciplinary collaboration involving clinical pharmacists provides a detailed follow-up of changes made: from reconciliation on admission to medication review with the development of a discharge summary document, transmitted to the general practitioner. The development of a geriatric reflex, regardless of the health care service, should be pursued, including updating and dissemination of our institutional practice guidelines in the elderly.
