Prévalence des différentes dimensions de précarité/vulnérabilité, retentissements et comorbidités associées chez des enfants et adolescents, en surpoids ou obèses, suivis au Centre Spécialisé Obésité (CSO) du Centre Hospitalier Universitaire (CHU) de Bordeaux entre 2014 et 2016

Abstract : Introduction: childhood obesity is a major public health concern because of its numerous short, medium and long term repercussions and comorbidities. Previous study have shown a link between obesity and precariousness / vulnerability. Our study aimed to determine the prevalence of the various dimensions of precariousness / vulnerability in overweight or obese children and adolescents. Méthods: the sample was composed of children and adolescents who were followed by the Specialized Obesity Center (SOC) of the University Hospital Center (UHC) of Bordeaux between 2014 and 2016. The dimensions of precariousness / vulnerability were defined on the basis of literature data and exchanges with professionals of the medical and social fields. These data and those for studying the repercussions / comorbidities of obesity for each child or adolescent were extracted from the UHC's medical records. Results: 252 children and teenagers were included in the study. Body Mass Index (BMI) at first stay was 32.80 (+/- 6.77) kg / m2 with a BMI Z score at 4.65 (+/- 1.36) SD. In our sample, 60.87% of children and adolescents had precariousness/vulnerability of economic dimension; 33.73% had precariousness /vulnerability of socio-educational dimension; 28.69% had precariousness / vulnerability of psychopathological dimension; 39.68% were precarious / vulnerable in relation to their scolarity and 50.00% presented a precariousness / vulnerability of health dimension (child and his family). Conclusion: most children in our population were in a precarious/vulnerable situation, in a multidimensional way and all had at least one repercussion of their obesity. All the actors of the medical and social field have to work together in order to have a global vision of the child in his family and social environment, in order to better identify his vulnerability and to improve the care management of his obesity.
