L'utilisation des tests rapides d'orientation diagnostique en médecine générale modifie-t-elle les représentations et les pratiques des médecins autour du thème du VIH ? Une étude réunionnaise

Abstract : Introduction: in France, 27% of diagnoses turn out to be done too late and 30 000 people ignore their seropositivity in France. Therefore, the Human immunodeficiency virus (HIV) screening is a public health issue. Rapid HIV tests are innovative tools that are not accessible to general practitioners. On the one hand, our study's goals consisted in exploring the changes in representations and practices of general practitioners, after using those tests. On the second hand, we aimed to collect a feedback of their using. Method: our study was a qualitative, prospective and interventional trial which included 15 practitioners of Reunion Island. The data collection consisted in individual and collective (focus groups) semistructured interviews, divided in two different analysis phases (before and after using the HIV rapid tests). The data analyse was based on the grounded theory methode. Results: 48 Rapid HIV tests were used within four months. They were all negativ. Some of the study attendees noticed changes in their representations and/or practices related toour research protocol. Rapid HIV tests provided a better mobilisation about HIV infection and had strengthened HIV screening. They made the raise of difficult subjects like sexuality and risk taking, easier. Using Rapide HIV tests exposed to practical problems, technical difficulties and frightening live-announcement. Financial aspect was a main limiting factor. Discussion: Rapid HIV tests associated to a specific training could be usefull tools in order to improve representations and practices of general practitioners. Those reflections should result in making rapid HIV tests more disposable in primary health care, minding a viable financial solution. Rapid HIV tests could facilitate necessary reappropriation of HIV infection by general practitioners like any other chronic disease, with strong issues of primary and secondary prevention.
