Évaluation de l'impact de l'intensification du programme transfusionnel chez les enfants atteints de béta-thalassémie majeure à Mayotte : une étude prospective

Abstract : Purpose: The aim of this study is to observe the consequences of a transient intensification of transfusion program of variation of hemoglobin and iron overload in thalassemic children treated in Mayotte, and evaluate feasibility and limits of this practice. Patients and Method: Seven patients with beta-thalassemia major were included. Blood transfusion intensification took place from January to April 2017, bringing the rate of transfusions closer to every 2 weeks until to get a hemoglobin level >9.5g/dl. Then, a usual rhythm of 3 to 4 weeks was resumed and the follow up was until February 2018. Results: Hemoglobin level was significantly higher after immediate intensification (median: 8.1g / dl VS 10.4g / l, p = 0.001) and in February 2018 after resumption of a usual rhythm (median: 8.1g / dl VS 8.4g / dl, p = 0.046). There was no significant difference in ferritin level after intensification (median: 557 ng / ml VS 1128ng / ml, p = 0.11). Conclusion: Intensification of blood transfusion seems to be indicated in these patients in order to obtain a hemoglobin level > 9.5g / dl, with no short-term impact on iron overload.
