Facteurs de risque modifiables de mort inattendue du nourrisson : état des lieux du suivi des recommandations à la maternité de Lisieux, évolution des pratiques au retour à domicile

Abstract : Introduction. One hundred and fifty cases of SIDS per year would be preventable. The aim was to take stock of the follow-up of maternity recommendations and to study their evolution at home, in order to better target the voices of improvement of practices. Material and methods. This was a two-phase observational prospective study: an observation of maternity practices from September 20 to October 28, 2016, followed by a telephone survey in the month following the return home. Results. Ninety-four cradles were observed in the maternity ward, of which 51 mothers responded to the home survey. A dangerous position was used for 1 in 10 infants in maternity and then at home. Coverage was inadequate for 43% of births in the maternity ward, with a significant improvement on return home (p = 0.04). Objects were present in 60% of the cradles of maternity, the evolution was favorable at home (p = 0.05). Mothers had used “cosleeping “ at 48% in the maternity ward, with a stable evolution of this practice at home. Finally, smoking affected 31% of mothers during pregnancy, 16% of them at home. Conclusion. In the maternity ward, every effort must be made to ensure that all newborns are fit for sleeping, and to stress the risks of "cosleeping" with a common speech. In outpatient care, we must accompany smoking cessation and ensure compliance with the recommendations during the critical period. Multimodal awareness should be encouraged to cover all modifiable risk factors in order to reduce the rate of avoidable deaths.
