Médecins généralistes et télémédecine en EHPAD : représentations, attentes et expériences

Abstract : Introduction: telemedicine appears to be a progress factor of the French healthcare system’s perfomance. The objectives of this study were to define representations, expectations and experiences of general practitioners in the field of telemedicine in nursing homes (NHs), identify bottlenecks and create leverages for better use of tool. Method: it was a qualitative study, using semi-structured interviews, with open thematic coding, analysis in anchored theorization and recruitment conducted in 3 Gironde NHs (rural, semi-rural and urban). We interviewed general practitioners with various uses of telemedicine. Results: 7 interviews were conducted between January 12, 2017 and April 4, 2017. Five themes emerged. (1) General doctor appears as multi-skilled, overworked professional ; elderly patients in NH require complex care. (2) Doctors, sometimes use a specialist opinion to reassure themselves. These exchanges are facilitated by networks of colleagues and digital means of communication, but sometimes it can be difficult because of the heaviness of the hospital functioning. (3) Telemedicine has to be promoted, but its use must be done under conditions. (4) Caregivers appreciate these new forms of interactive professional exchanges, which promote formation, strengthen bonds between colleagues, fight against isolation of the liberal and geographical distance. Patients save time, money and gain comfort by accessing new efficient therapeutic proposals. (5) Telemedicine finds its place but must be used reasonably, the general practioner remains the pivot of patient's care path. Conclusion: telemedicine brings a positive solution to the future of medicine as long as it remains at the service of human activity.
