Conformité de la réhabilitation précoce après césarienne : étude rétrospective sur dossiers aux hôpitaux Drôme Nord site de Romans

Abstract : Objectives. Enhanced rehabilitation after surgery (ERAS) in the context of cesarean section is a practice that exists in 64% of French maternities. In the HDN maternity in Romans, an ERAS program has been put in for a year. The study objective was to conduct an evaluation of professional practices after the implementation of this protocol. Material and methods. A descriptive, observational, retrospective, single-centric study based on ERAS patient records after a prophylactic caesarean section between the 1st, 2016 and May the 30th, 2017 at HDN was conducted. The main objective of the study was to assess compliance with the service protocol. The secondary objectives were to estimate the average length of hospital stay (ALHS) and to count the number of complications after a caesarean section. Results. Regarding the basic principles of ERAS, 36.4% of patients were able to mobilize within six hours after cesarean section. 30% of the patients were quickly able to eat and drink. The removal of the urinary catheter was performed for 27 patients (75%) and the venous catheter for 10 patients (27.8%). The ALHS was 4.25 days (+/- 0.87 days) and two post-operative complications were identified (only one in relation to ERAS was estimated). However, it should be noted that 48 out of the 75 items in the protocol (64%) had an unrecovered data percentage superior or equal to 20%. Conclusion. The results obtained during the study revealed a lack of clarity of the service protocol as well as a lack of traceability of acts of care and organizational dysfunctions. Thanks to the results of the study, the protocol has been updated and a second evaluation of professional practices is planned after having raised the awareness of the multidisciplinary team.
