Impact des maisons de santé pluridisciplinaires dans la vie et la pratique des médecins généralistes en Normandie orientale

Abstract : Facing a growing demand for care in Eastern Normandy, Multidisciplinary Health Centers (MHC) offer a new practice in general medicine. Eastern Normandy has to deal with medical desertification both linked to the ageing of the general practitioners (GPs) and its population.
The study aims at exploring the impact of MHC exercise in the life and practice of GPs settled in Eastern Normandy.
The data collection took place in the Eure and Seine-Maritime counties from October 2017 to July 2018. Based on semi-structured interviews until data saturation, this qualitative survey was recorded with the agreement of the general practitioners. The data transcription was performed on the Numbers spreadsheet. The number of twenty general practitioners led to the data saturation.
Among their motivations, general practitioners seek a group exercise to pool resources and skills, to arrange their working time and for some, it even represents a public health issue. Few GPs admitted some fears prior to integrate a MHC. The conviviality that prevails in these structures ensures better conditions of exercise, and upstream, an improvement of their quality of life. Their exercise is considered as safer for both practitioners and patients because it enables a real coordination of care provided to the patient. However, the costs can be a barrier to join a MHC. The interviewed GP’s refer to high costs balanced by a better quality of life and work. The pointed limits are the difficulties to work as part of a group and the conflicts that may occur. Administrative workloads are related to the setting up of projects, which represent the basis of these structures. Projects are led by "leaders" or coordinators. The consulted literature confirms the results of our study.
The exercise in MHC is booming, and their attractiveness allows little by little to fight against medical desertification. These structures contribute to perpetuate the profession. A dynamic group management and a harmonization of practices are the main conditions of its proper performance.
