Évaluation du traitement par trioxyde d’arsenic dans la prise en charge des leucémies aiguës promyélocytaires de l’enfant : tolérance et devenir

Abstract : Background: Since 2015, the treatment of standard-risk acute promyelocytic leukemia (APL) in children in France is based on arsenic trioxide (ATO)/all trans-retinoid acid (ATRA) without chemotherapy, or on the ATRA/chemotherapy combination. Although ATO treatment has been used since 2015, there are no studies evaluating its tolerance in children and adolescents treated for APL and their future. Purpose: Objective of our study was to evaluate the tolerance and occurrence of ATO-related side effects in children treated in the centers of Société francaise de lutte contre les cancers et les leucémies de l'enfant et de l’adolescent. Results: Twenty-one patients were treated in seven reference centers. Among them, 3 patients had high-risk acute promyelocytic leukemia with white blood cells greater than 10 G/L. A short discontinuation of ATO was required for 3 patients. Some side effects (Grade ≤ 3) were observed, according to the Common Terminology Criteria for Adverse Events. All patients were in cytological remission after induction. All patients achieved molecular remission during treatment. Overall survival and relapse free survival with mean follow-up at 17 months was 100%. Conclusions: Therapy with ATRA and ATO in our pediatric standard risk patients with APL was well tolerated without serious adverse effects. This study was promising for the management of high-risk promyelocytic leukemia, but required longer-term follow-up.
