Enfoncement d’une tige fémorale non cimentée dans les arthroplasties de hanche pour fracture du col fémoral après 65 ans : évaluation clinique et radiologique à 3 mois d’une cohorte de 117 patients

Abstract : Introduction: The use of a cementless femoral stem in hip arthroplasty for femoral neck fracture in a geriatric population is discussed. There is a chance of early subsidence. The main objective of this study was to measure the subsidence of an uncemented femoral stems at 3-month, in treatment of a femoral neck fracture, in a population over 65 years old, and its risk factors. The secondary objective was to assess the relationship between subsidence and dislocation in the first 3 months. Material and method: We retrospectively evaluated the patients treated for a femoral neck fracture who underwent arthroplasty using an AVENIR-MULLER stem (Zimmer-Biomet company), with a radiological follow-up up to 3 months postoperatively. Subsidence was measured according to the technique of Engh and Massin, with two modalities: in absolute value and in groups depending on whether the depression was ≥2mm or <2mm. Results: We evaluated 117 patients, mean age 83 years (65 to 97). The average subsidence at 6-weeks was 1.1 ± 1.5 mm (0 to 9.1), and at 3 months 1.4 ± 1.6 mm (0 to 9.2). At 3 months, 22 patients (19%) had a subsidence ≥2mm. A dislocation occurred for 9 patients (8%), without correlation with the subsidence. Discussion: Our study evaluated the subsidence of an uncemented femoral stem in a geriatric population, in a traumatic context. The aging of the patients did not show any relation with the subsidence. There was no evidence of correlation between subsidence and the occurrence of prosthetic dislocation. A Cortical Thickness Index ≤0.40 seems to be associated with a greater subsidence.
