Évaluation des performances de l'IRM en post radio-chimiothérapie concomitante pour la prédiction du pronostic des cancers du col de l'utérus localement avancés

Abstract : Objective: To evaluate the performances of magnetic resonance imaging (MRI) in predicting the long-term clinical outcome of patients with locally advanced cervical cancer after concurrent chemoradiation (CT/RT). Methods: A multicentric retrospective study was conducted including 216 patients presenting FIGO IB1, IIA and IIB cervical cancer for which post treatment MRI was performed after the completion of CT/RT. All patients underwent regular follow-up. Partial response was defined as persistent cervical and/or nodal lesion on post treatment MRI. Results: 102 (47.2%) patients had partial response on post-treatment MRI. A partial response was predictive of a poorer overall survival (HRa : 2.30 ; CI: 1.00-5.40 ; p=0.047). Other prognostic factor was pre-treatment nodal status for both recurrence-free (HRa : 2,90 ; IC à 95% : 1,60-5,20 ; p < 0,001) and overall survival (HRa =2,70 ; IC à 95% 1,10-7,10 ; p=0,04). Completion surgery was performed in 117 (54.4%) patients and residual disease was identified in 55 (47.4%) cases. Completion surgery did significantly effect recurrence-free and overall survival in both patients with partial metabolic response. Post treatment MRI sensitivity and specificity in predicting residual disease in the cervix were respectively 75% (CI: 63-86) and 51% (CI: 38-63). Conclusion: Partial response on post treatment MRI after completion of CT/RT in patients with locally advanced cervical cancer in predictive of a poorer overall survival.
