Prise en charge précoce de l’obésité pédiatrique : définition et impact sur les résultats de prise en charge. Étude de 1510 enfants et adolescents dans le RéPPOP Aquitaine de 2006 à 2015

Abstract : Background: Childhood obesity has been recognized as a major public health issue. The importance of early detection of overweight and obesity in children has been highlighted in many recommendations. The main objective of this study is to assess the impact of an "early management" of pediatric obesity, on the BMI Zscore for children monitored within RéPPOP Aquitaine. Population and methods: Materials and methods: 1510 children, aged 2 to 18, who received care in RéPPOP Aquitaine between January 2006 and January 2015 were included. We created the variable "management delay" (duration in years between the beginning of the onset of overweight and the initiation of care in RéPPOP); care is defined as "early" if the child has a management delay strictly lower than the median of the management delay for his age group and as "late" if the child has a management delay greater than or equal to the median of the management delay for his age group. Results: 1510 children aged on mean 10.5±3.1 years were included, 58.5% were girls and 59.5% were obese at inclusion. The mean BMI Zscore at inclusion was 3.48±1.05 and 3.10±1.13 at the end of care. 77.2% of patients had decreased their BMI Zscore. Early management delay was significantly associated with a significant decrease in relative ∆BMI Zscore in both univariate (-0.12±0.21VS-0.10±0.17) and multivariate (-0.061[-0.111;-0.012] analysis. Conclusion: Early management of pediatric obesity, within RéPPOP Aquitaine has a positive impact on reducing the corpulence of children.
