Utilisation de la capsaïcine dans le traitement de la douleur neuropathique

Abstract : Neuropathic pain is defined as pain associated with injury or dysfunction of the nervous system. The analgesics of the three levels and the anti-inflammatory remain little effective to treat these pains. The treatment of these latter uses other molecules such as antidepressants and antiepileptics. Capsaicin, a pepper molecule derived from a plant belonging to the genus Capsicum, is found in a drug called Qutenza®. It is the active ingredient of this transdermal patch with an indication in the treatment of peripheral neuropathic pain in adults. The work carried out within this thesis has demonstrated the mechanism of action of capsaicin which binds to TRPV1 receptors. They also justify why neuropathic pain treatment recommendations place Qutenza® as second-line and above. This thesis also studies the interest of capsaicin beyond the treatment of neuropathic pain. Its neuroprotective properties, anti-cancer, its potential interest in the treatment of cannabinoid syndrome, in weight loss and in the treatment of allergic contact dermatitis are developed. Finally, this work presents molecules derived from capsaicin currently under study, such as olvanil, arvanil and NEO6860. Their activity would not be limited only to neuropathic pain, but would extend to other chronic pain, and as an anti-cancer agent.
