Optimisation de l’utilisation du plerixafor : étude visant à identifier les facteurs de mauvaise mobilisation de cellules souches hématopoïétiques en vue d’une autogreffe

Abstract : Autologous hematopoietic stem cell transplantation (HSC) is a type of transplant whose cells to be transplanted come from the patient himself. During mobilization and collection of HSC, there is a large interindividual qualitative and quantitative variability of HSC, whose causes are still not clearly established. We studied the influence of various factors on HSC mobilization and its collection from a cohort of patients from the University Hospital of Toulouse. Medical data were collected retrospectively for patients who benefited from HSC collection between 2014 and 2016. The influence from various factors was assessed on two main parameters: HSC mobilization and its collection. Secondarily, HSC loss during transplant defrosting and the exit delay of agranulocytosis were analyzed. After univariate and multivariate analyzes of selected factors, a linear regression model was established for each parameter. 319 patients were assessable. Interindividual variability in mobilization is poorly explained by patient characteristics, even if BMI, Rituximab, and indolent NHL diagnosis appear to have a positive influence on mobilization, and previous treatment with Lenalidomide, radiotherapy exposition, LH diagnosis, and female sex appear to have a negative influence on mobilization. Regarding the HSC collection, its quality is improved by the circulating CD34+ rate and the use of Cyclophosphamide for mobilization, while an induction containing IMID (Lenalidomide and Thalidomide) seems to lower its quality. These results enable us to target factors which need to be taken into account for HSC collection optimization. However prospective studies remain necessary to validate their influence.
