Impact de la démographie médicale en Dordogne sur les passages aux Urgences du Centre Hospitalier de Périgueux : étude prospective réalisée du 17 octobre 2016 au 30 octobre 2016 à propos de 787 passages

Abstract : Introduction: the Dordogne is one of the largest medical desert in the country with a staff of general practitioners reduced by 14% since 2007. In parallel, in France and Dordogne the number of emer-gency unit admissions continues to increase with a rise of 3,5% on average per year. Objectives: to evaluate the impact of medical desertification on patients attending emergency de-partments and determine the modalities for emergency unit visits. Material and method: this is a prospective study conducted using a questionnaire completed for any patient over 18 years, not recalled, coming to the emergency department of the hospital of Périgueux between 17 and 30 October 2016 Two groups of medical demographics were defined: high (> 8MG / 10,000 inhabitants) (DME), low (<8MG / 10,000hab) (DMF). Results: 787 questionnaires were analyzed. 65% of the patients were from the DME group, 78.5% of whom lived within 15km of the hospital. Patients in the DMF group are proportionally more "severe" but those in the DME group come more directly to the emergency department (61.3% vs. 38.7%, p <0.001). 10.9% of patients from an DME zone have no registered physicians, compared to 4.8% in DMF. Medical demographics do not affect the schedules and days of consultation. Conclusion: the medical desertification does not directly affect the passages to the emergencies, on the contrary, the patients of the desert zones have more medical contact prior to their arrival where-as the patients of the strong medical demographics, living near the emergencies, will go there more directly.
