Durée de l'intervention : 420 min Pertes sanguines : 800 cc Transfusion : 2 culots globulaires Caractéristiques post-opératoires en réanimation : Hémodynamique : stable, Pression artérielle 94/55, FC = 71, hypothermie 34.6°. Digestif : Ictère-Echographie hépatique : pas de dilatation des voies biliaires. Transit + Abdomen souple, dépressible et indolore. Drains Trans cystiques en place Drain de Shirley : 800 cc claire, quotidien en baisse Coeur : pas d'insuffisance cardiaque. Pulmonaire : eupnéique sous 2l/min d'O2. Epanchement pleuraux bilatéraux de moyenne abondance (radiographie thoracique et échographie pleurale). Rénal : Oligurie avec insuffisance rénale (Cl = 60ml/min) fonctionnelle, remplissage vasculaire. Puis hyper diurèse réversible en 2 jours. Neurologique : RAS Biologie Acidose métabolique à trou anionique augmentée, Autres données : clampage systématique 15 puis 5 min. Foie compatible avec la volumétrie et sain. Anatomie biliaire non modale. (Interruption de la convergence biliaire avec les canaux du II et du III en arrière et au-dessus de la veine porte gauche et du ligament rond. Pas de fistule biliaire
