Que pensent les patients de l’interchangeabilité d’un médicament biologique vers un biosimilaire ? Enquête auprès de patients chroniques traités par infliximab

Abstract : The introduction of biosimilars is both an health and economic issue. In this context, biosimilars appear as an option to maintain patients' access to innovative therapies by reducing health expenditure. The agreement of the patient to consider an interchangeability of the original biomedicine to the biosimilar is unavoidable and requires knowledge of its positioning. The purpose of this survey was to gather the opinion of patients on a proposal to change their usual biomedicine for a biosimilar, in order to identify the potential barriers and levers for their acceptance and to consider the educational actions to accompany the change. This survey was conducted in 2016 among 106 chronic patients treated by infliximab. The results show in the first place the lack of knowledge of patients on the notion of biosimilar. After a standardized explanation of biosimilar, more than half of the patients would accept the switch. The reasons for an agreement are mainly based on an agreement in principle followed by the argument of economic gain and the similarity of the molecule. One-third of patients refuse the switch, mainly because of a status quo posture linked to their strong attachment to the original biomedicine followed by a fear of a loss of efficacy. About 16% of patients have an ambivalent position. They need to be reassured about tolerance and rely on a convincing opinion from a convinced medical specialist. Clear and transparent information about biotherapies and biosimilars therefore seems necessary so that patients can confidently adhere to the switch. This underlies a harmonization of the messages transmitted by all health professionals including the pharmacist.
