Retentissement fonctionnel du traitement par oxygène hyperbare après accident vasculaire cérébral en phase chronique : étude prospective sur 8 cas

Abstract : BACKGROUND: Stroke is a major public health issue that generates mortality and high costs for society. The therapeutic management of stroke in the chronic phase remains of limited effectiveness with a low proportion of convincing results in functional rehabilitation. Hyperbaric oxygen therapy (HBOT) as adjuvant therapy appears to be an interesting alternative by induction of neuroplasticity. The objective of the study was to evaluate the effect of HBOT on performance in terms of activities and participation in the chronic phase of stroke.
METHODS: The study is prospective and monocentric. The subjects included were major and had ischemic or haemorrhagic stroke occurring between 6 and 36 months before the inclusion, responsible for motor impairment. They were followed during two successive phases differentiated by the administration or not of OHB and sanctioned each one by a day of evaluation on the activities and participation. Subjects received 100% oxygen administered at 2,5 absolute atmospheres for 90 minutes over 40 sessions. The respective evolutions of the subjetcs were compared between the two phases.
FINDINGS: The study included 8 subjects (1 excluded). The distance and walking speed at the 6-minute walk test were improved without significance criteria in both phases. The improvement was greater during the "not OHB" phase. Neurological functions, grasping activity and functional scores remained stable during the study.
CONCLUSION: The results show a potential for improvement of walking activity at the chronic phase of stroke by the administration of HBOT. It is important to build controlled, randomized and well-conducted studies to confirm these findings.
