Les groupes d'analyse de pratiques en 3ième cycle de médecine générale : étude qualitative de leur apport dans la formation à la gestion de l'incertitude diagnostique

Abstract : Context : Medical proceedings are the basis of diagnostic and decisional steps, leading to make decisions without settled diagnosis, in a very short time with limited technical means, taking into account the environmental and psychosocial pressure. Objective : In order to estimate the interest of Practice Analysis Groups (PAG) in the teaching of the General Practitioner's Diploma in the training to the managing of diagnostical uncertainty. Method : It is about a qualitative descriptive investigation using a general inductive approach, made among house physicians in general practice and young general practitioners, who had already taken part in PAG, from semi guided interviews and from a focus group, all led to data saturation. Results : 14 young doctors were interviewed from February to August 2017. The analysis of this data made 2 major themes emerge : the vision of young doctors in a situation of diagnosis uncertainty and the importance of teaching through PAG in these situations. A natural link between PAG and the problems met in real practice has emerged and made of PAG an interesting pedagogical tool as far as the managing of diagnosis uncertainty is concerned, through its psychological support, its group experience, and its approach of the general practice specificity. Conclusion : The managing of diagnosis uncertainty in general practice is linked to many fields in the context of PAG, both on the intellectual and the emotional levels. They contribute to the student's steps to a complete autonomy as a general practitioner through a mastering of a reflexive autonomy, the learning of communication skills and the aspects that characterise general practice. This mastering is made through a complex medical scheme allowing a holistic approach of the patient in a reassuring environment promoting a psychological support.
