La césarienne de convenance : motivations, représentations et facteurs décisionnels

Abstract : Background: The frequency of caesareans has significantly increased over the last 20 years. Of the 800,000 births per year in France, one in five is performed by caesarean section. Today this rate is 20.4%, it has stabilized since 2010. This caesarean section inflation occurred despite the WHO recommendations of 1985, which advocate not to exceed a threshold of 15%. This increase is mainly due to an increase of the age of the pregnant women causing more complications during the pregnancy (HTA, DG, PE, prematurity, IUGR...), more recourse to the techniques of PMA and more pregnancies multiple. All this associating with societal changes (freedom of decision, self-determination, autonomy of patients) increasing the maternal demands of caesarean section without medical indication. Objectives: Identify the representations and motivations of pregnant women who choose a caesarean without medical indication. Identify the decision-making factors which motivate pregnant women to request an elective caesarean section Methods: Multicenter qualitative study with semi-directive interviews performed after the birth of caesarean section, until saturation of the data. Results: Tokophobia and the need to preserve the baby are the two main motivations. The information is mainly collected on the internet and based on the experience of deliveries of others, the health professional intervenes only very little in the decision-making. Conclusion: We need to strengthen the health carer-patient relationship in terms of trust, in order to facilitate the collection of information from health professionals.
