Évolution de l'asthme vers le recouvrement asthme-BPCO : approche pharmaco-épidémiologique dans une population de femmes âgées

Abstract : Even if asthma and COPD are initially defined as two very different entities, it is now well recognized that these two diseases can overlap and define the Asthma-COPD Overlap (ACO). The objective of this thesis was to identify women moving from asthma to ACO by the analysis of drug reimbursement databases. This longitudinal study, based on a pharmacoepidemiological approach, was conducted from 2006 to 2013 among elderly women with ever asthma belonging to the Asthma-E3N study. We used longitudinal drug reimbursement data of the MGEN database for identifying asthma evolution towards ACO, based on differences in therapeutic management between asthma and COPD and on the dynamic of treatment consumption over time. The associations between ACO onset and determinants were estimated by logistic regression. Our study population consisted of 4,240 elderly women with ever asthma in 2006. 249 women with ACO were identified. The risk of ACO onset was significantly higher among obese women than among those with a normal BMI (aOR [CI95%], 1.79 [1.21-2.64]), among current smokers than among never smokers (aOR [CI95%], 3.30 [1.96-5.67]) and among women with late-asthma onset (≥ 40 years old) (aOR [CI95%], 1.44 [1.04-1.98]). No association was found between ACO onset and education level or rhinitis. Our results being consistent with those read in the literature, our study supports the pharmacoepidemiological approach for the identification of people moving from asthma to ACO.
