Apport de l’étude des cas cliniques abordés en groupe d’échange et d’analyse des pratiques : à propos de six groupes constitués de jeunes médecins généralistes remplaçants en Nouvelle Aquitaine

Abstract : Objectives: several studies have shown that peer grouping is an expert group, each providing his skills, in order to solve problems encountered in consultation. It would seem, therefore, that this type of continuing medical formation has a positive impact on practice. Main objective: to know the contributions in practice "about cases" presented in Peer Groups (GPs) constituted of locum general practitioners between one and six years after the DES. Method: qualitative study by semi-directed individual interviews analyzing a clinical case discussed in a previous GPs meeting, with locum general practitioners, practicing in Nouvelle Aquitaine, one year after their DES. Results: 13 individual interviews were conducted from 6 different GPs. Each clinical case approached had an impact on their practice, gathering different themes, regardless of the role of listener or actor. In 2 groups, exchanges via the application WhatsApp® exist to extend the link outside the sessions, allowing sharing and questioning other cases experienced. The specificity of locum practitioner is the lack of feedback on their practice, the fear of forgetting knowledge, as well as a particular patient-locum general practitioner relationship requiring an adaptation of their practice to the patient and to the replaced doctor, assisted by the GPs. Initial formation appears to be the starting point for these GPs as 10 interviewees were sensitized during their initial formation and wanted to replicate this experience. A pedagogue profile is present for group’s leaders. None of the 6 GPs is labeled to validate the Continuing Professional Development. Conclusion: meetings are experienced as accessible and friendly meetings. The sharing of the practice and the social link motivate the participants to form regularly, as an extension of the initial formation.
