Le ressenti des femmes faisant leur frottis chez le médecin généraliste en milieu rural : freins, motivations et motifs de changement d’opinion quant à l’acceptation de ce suivi

Abstract : Object: screening for cervix cancer is marked by significant inequalities of access. In some rural areas, specialized consultations with gynecologist can be inaccessible for most of the womens due to the remoteness. The study took place in rural areas of the department named Landes, in a medical center where general practitioners practice smears. The aim of our study is to analyze the pros and cons for women to take their smears at their GP. Material and Methods: qualitative study with semi-structured interviews. 14 patients aged from 25 to 65 years old and making the smear at the medical center were included. Results: the main motivating factor for women is the convenience of access to consult with their general practitioner compares to the remoteness of the gynecologist practices. The skills of their general practitioner and his or her ability to delegate when it is necessary increases the patient confidence. Their human qualities, attentiveness, reassurance and sometimes a privileged relationship, are very important for women. The main inconvenients are modesty in front of a male doctor, the fear of the smear itself and its result, as well as the preference for a gynecologist who is sometimes considered more qualified for complex situations. There is a lack of information about smear, but also about the role of general practitioners in this screening. Conclusion: to recall the importance and to limit the existing brakes about the smear, the results suggest that there is more attractive information, and to have a more systematic proposal of the practitioner to carry out the smear, as well as the creation of a system for follow up visites for the patients. The results of the smears could also be simplified to limit the women anxiety. A poster in the waiting room stating that the doctor carry out smears has a significant impact and could be generalised in other practices.
