Scolioses idiopathiques d’angles inférieurs à 25° : dépistage et prise en charge en médecine générale

Abstract : Context: idiopathic scoliosis is the most common type of spinal deformity with a prevalence of 2 to 3% in adolescent age group. The probability of evolution and sometimes serious complications render early detection and regular medical supervision essential. Though the cases of mild scoliosis are most frequently first seen by general practioners, the treatment of mild curvatures of the spine has not been the subject of recent recommandations. Objectives: this includes carrying out a litterature review to reach consensus on IS management especially concerning cases in which the Cobb angle is inferior to 25°. Then establish algorithms to help guide general practioners in overall primary care of these cases. Methods: we have therefore constituted a summary of thesis which evaluate the therapeutics used by GPs concerning the protocols of managing patients with IS. The data concerning detection, diagnosis, prognostic factors, treatment and the follow-up of the IS patients, insisting on those with a Cobb angle inferior to 25° were the subject of the synthesis of the litterature. Results : the studies carried out among the GPs show a lack of homogeneity in the handling of IS patients. The detection of this pathology is in fact generally done by GPs. Nevertheless, we noticed a high tendency to refer to a specialist for the follow-up regardless of the angle of curvature. The synthesis of the littérature show that the detection, diagnosis as well as the follow-up care of the IS of less than 25° was left to the care of the GPs and that the necessity to call in a specialist was necessary only in cases of untoward evolution (or severe curvature superior to 25°). Conclusion: the two algorithms established in this thesis should help improve the knowledge of GPs on the subject of IS and guide them in their daily practice so that they can offer their patient an adapted protocol.
