Recherche d'un lien entre le taux de vitamine D et les complications du diabète gestationnel

Abstract : Background: Vitamin D deficiency is a common issue among general population and especially pregnant women. Studies have suggested the relationship between gestational diabetes mellitus (GDM) and vitamin D status. However, this relationship remains unclear. We evaluated the association between vitamin D levels and the need to introduce insulin therapy, as well as the occurrence of complications. Methods: a total of eigty-seven volontary pregnant women with GDM have been enrolled in this prospective analysis between February 2017 and March 2018. The patients were divided in two groups : vitamin D deficiency (< 30ng/ml) and normal rate (> 30 ng/ml). When insulin was needed, term at introduction and insulin doses were recorded. Pregnancy parameters as weight gain were compared between these two groups, and delivery parameters were analysed (baby's weight, delivery or neonatal complications). Results: incidence of vitamin D deficiency (< 30 ng/ml) was 66.6 %, and normal rate was 33.3 %. There was no statistical difference between the two groups regarding introduction of insulin therapy (p = 0.45). However, patients in the "deficiency" group were diagnosed with gestational diabetes earlier than the "normal rate" (p = 0.033). Nevertheless, insulin therapy was introduced earlier in the "deficiency" group (p = 0.048). In the “deficiency” group, there were 12.1 % fetal complications, and 6.9 % in the “normal rate” group. There were 1.7 % maternal complications for the normal rate, compared with 10.3 % in the deficiency one. Conclusion: these results suggest that vitamin D deficiency may have a negative impact in the gestational diabetes control and lead to an earlier insulin resistance.
