Évaluation des pratiques professionnelles : impact d’une sensibilisation sur l’amélioration de l’hygiène des mains aux urgences, étude faite aux urgences du CHU de Pellegrin en 2017

Abstract : Introduction: in the daily practice of medicine and especially in emergencies, hand hygiene is essential in the fight against hand-borne infections. Healthcare-related infections affect 5 to 15% of hospitalized patients and their prevalence is 15.5%. Friction of hands with an alcohol-based hand gel is the preferred method in most common clinical situations., if the activity is more important, the compliance of handwash decreases more, which explains why hand hygiene is less well performed in emergencies. The main objective is therefore to determine whether awareness of hand hygiene would improve practices in an emergency department. Materials and methods : this was an evaluation of professional practices : observational study, prospective, mono-centric, analytical conducted in two stages from March to April 2017 with an intervention between the two phases of observation. The data was collected on the basis of observations made at the Pellegrin hospital emergency department of the Bordeaux University Hospital by means of a questionnaire. The results are expressed as a percentage. Results : 674 observations were made and the intervention reached 89 participants. The results of this evaluation were positive. There is an improvement in the compliance of handwashing after intervention, and this significantly both at the box entrance and at the exit : respectively 40% vs 53% (p = 0.01) and 47% % vs 80% (p <0.01) The quality of the friction also improves significantly with the box entrance 35% vs. 47% (p = 0.01) and 41% vs 66% (p = 0.05) at the exit. Conclusion : hand hygiene in hospital services is a simple gesture but remains the most important measure to limit the transmission of germs. It may be appropriate to provide awareness periods regularly to give a booster to the medical and paramedical teams. Indeed, the improvement of hand hygiene requires the training and information of the staff who is the first player in the reduction of hand-borne infections.
