Vécu de l’annonce de la maladie grave par le patient à La Réunion : étude qualitative

Abstract : Introduction: the announcement of a serious illness is an information which will affect the conception of life for the patient. The information is an essential element of the physician-patient trusting relationship. The main purpose of this study was to explore the lived experience of the patient toward the announcement of their illness which they qualified as serious in Reunion Island. Material and Method: it was a qualitative study with semi-direct individuals interviews, with patients consulting at a generalist office. The data analysis was made with the grounded theory method. Results : Six patients were interviewed between November 2016 and December 2017. The lived-experience of the announcement was evolving, not freezed, but enshrined in a period, starting at the apparition of symptoms and ending at the understanding of the illness. There were internals factors (patient’s life story) and external factors (physicians, treatments, familial and socio-professional environment) which influenced the experience of the announcement of the serious illness. We’ve noticed positives elements regarding the experience : relief, willing to cope, hope of recovery. The negatives points found were sadness, fear, anxiety and wish to die. Patients regretted the lack of information and also the lack of understanding of medical terms. However patients didn’t want to change anything about the announcement and appreciated the delicacy and the honesty of the doctor announcer. Conclusion: the serious illness announcement is enshrined in a temporality. Thus, the patient lived experience evolves according of steps which brings him to understand his illness and accept it. This results point the importance for the physician to adapt with the patient psychic and cognitive disponibility in each constituent step of the announcement.
