Quelles sont les modalités de l’accès aux soins de santé primaire chez les gens du voyage, point de vue des patients : étude qualitative menée auprès de patients issus des communautés de gens du voyage de Gironde

Abstract : Introduction. Since they moved from northern India in the 10th century, Gypsy populations have been stigmatized and have lived precariously on the edges of society. Due to their unique way of life and culture, the issue of health is not a primary concern leading to a life expectancy of 15 years lower than that of the general population. The purpose of this work was to identify ways of accessing primary care among Travelers, in order to suggest ways to improve. Method. We conducted a qualitative study through semi-structured interviews with people belonging to the French Traveler community, who were then transcribed and analyzed according to the method of the grounded theory. Of the 10 interviews conducted, we interviewed 4 women and 6 men with an average age of 46.5 years old. The interviews lasted on average 16 minutes and 21 seconds. Results. The main recourse evoked by the interviewees was the referring physician, with whom a relationship of trust, mainly based on communication, is established over the long term. It is requested for acute diseases, especially in children, for the monitoring of chronic diseases, but very little for prevention. In case of unavailability or during travel, Gypsies call on other private doctors, the doctor on call or SOS doctor in case of closing of medical offices, or directly to the emergency services. Nevertheless, the use of unknown doctors is a source of fear and anxiety, and preference will always go to the referring physician. Discussion. We have thus seen that access to care for Travelers is sometimes difficult and a source of anxiety for these populations. But there is still much research to be done to continue improving care within this population.
