Effets neuroprotecteurs et neurotoxiques des thymorégulateurs dans le trouble bipolaire

Abstract : Cognitive deteriorations are responsible for a major handicap in bipolar disorders. There would be cognitive deteriorations inherent in bipolar disorders resulting from the physiopathology of the disease: attention, verbal memory and executive functions would represent the cognitive functions that are the most damaged among patients suffering from bipolar disorder. These cognitive disturbances would worsen with disease evolution, and would tend to increase with:time of disease, the number of manic episodes, psychotic symptoms. Pharmacological treatments could also explain the cognitive disturbances related to the disease and the question of their implication arises. The objective of this work was thus to precisely identify the cognitive impact of lithium, valproate, carbamazepine and lamotrigine in the bipolar disorders. For that, we carried out a literature review. The cognitive impact of mood stabilizers is very variable from one study to another. Lithium seems to have a better neurocognitive profile by involving an improvement of attention and executive functions, and to slow down the cognitive decline related to the age with elements in favour of a prevention of Alzheimer’s disease. Lamotrigine would be responsible for an improvement of executive functions and would involve less deteriorations on attention and memory than valproate and carbamazepine. Co-medication associated with mood stabilizers in bipolar disorders (antipsychotics, antidepressants, benzodiazepines) would have bad cognitive effects, knowing that the impairment on verbal memory would be associated to polymedication. A neurocognitive evaluation among patients suffering from bipolar disorder is of primary importance for pharmacotherapies.
