Psoriasis génital : prévalence, description séméiologique et impact sur la qualité de vie chez les patients consultant pour un psoriasis extra-génital : étude observationnelle prospective multicentrique GENIPSO

Abstract : Background: genital psoriasis is often under-recognised. Objective: To assess instantaneous prevalence of genital psoriasis, describe its clinical features, association with a subtype of psoriasis and its impact on general and sexual quality of life (QOL). Methods: GENIPSO is a prospective study conducted by private and hospital-based dermatologists: -consecutive inclusion of patients consulting for extra-genital psoriasis; -capture of clinical features of psoriasis and genital psoriasis; -distribution of QOL and sexual health questionnaires to patients. Results: out of 776 patients included, 335 (43.2%) had genital involvement. All were aware they had genital lesions but only 135 (40%) declared having been previously examined. Genital lesions were associated with male gender, severity of psoriasis, age of onset over 20 years, inverse psoriasis and involvement of scalp, nail and external auditory canal, but not with obesity, psoriatic arthritis and active sex life. Itching was the main symptom. Genital psoriasis was associated with impairment of QOL and sexual health according to the Dermatology Life Quality Index, the Female Sexual Function Index. Conclusions: genital psoriasis has a high prevalence in patients consulting for extra-genital psoriasis, affecting QOL, and should be better taken into account by dermatologists for optimizing global care.
