Facteurs prédictifs d'une rechute à un an chez une cohorte de patients hospitalisés pour un premier épisode psychotique sur un secteur parisien

Abstract : This retrospective study sought to find predictors of relapse within the year following a first episode of psychosis hospitalized on a Parisian psychiatric ward. Beginning abruptly at young adulthood, with a still nonconsensual definition, consequences may lead to a severe burden for the patient. Each relapse grieves the functional outcome, majoring social impairment and suicidality. Our main criterion on judging was to find a predictor of relapse associated with the care project. Our secondary criterion on judging was to find à predictor of relapse within the premorbid state of the patient. To answer these questions, 78 criterions have been collected on a sample of 46 patients who have been diagnosed with a first episode of psychosis and been followed for at least for one year. Principals results of this study are an increased risk of relapse with the lack of linked with the general practitioner, and with the necessity of a high social follow-up inside the hospital ward. Theses originals results are compatibles with the recent finding of the literature, pointing an insidious prémorbide evolutionary state with cognitive impairments prior to the first episode of psychosis. A situation that could explain both of these hypotheses. Finding a way of preventing a relapse after the first episode of psychosis is still a major issue. Inspired by the continuous growth of interest about early intervention programs, coordination of care between the hospital and the general practitioner, as well with the medico-social services seems to be relevant and beneficial for the long-term evolution for the patient.
