Anomalies de l’expérience de soi et troubles de la mémoire de source dans les phases précoces de la maladie schizophrénique : y a-t-il un lien ?

Abstract : Over the past twenty years, countless works on early psychosis have emerged and enabled us to define several fundamental concepts for early detection of schizophrenic pathology for a more rapid and more targeted intervention. Among these concepts, the Parnas’ team defined the disturbances of the basic sense of the self in schizophrenia and their exploration by the EASE scale which consists in a very complete semi-structured interview and exploring the whole range of disorders in question. Studies have shown that these disorders are highly specific to schizophrenia and present very early in this pathology, from the stage of early prodromes; these disorders are therefore the hard core of schizophrenic symptomatology and make it possible to better delineate their clinical characteristics. Thanks to this delimitation, a better exploration of underlying cognitive disorders can be carried out. According to Nelson (2014), and given the data available today, the disturbance of the basic sense of the self in schizophrenia would be related, at the cognitive level, to finer and more specific disorders than those explored by standard cognitive test batteries, such as, for example, source monitoring deficits, also specifically found in schizophrenic patients (and especially in patients with acoustico-verbal hallucinations) and early.
This is the hypothesis of this work: the main aim is therefore to see if there is a correlation between these two parameters in schizophrenic patients at the beginning of the disease. As secondary aim, the impact of passing the EASE scale on the therapeutic alliance and on the insight is evaluated. The results of this exploratory study suggest a strong correlative trend between disturbances of the basic sense of the self, measured by the EASE, and source monitoring deficits, measured by the source monitoring test of Brunelin et Al. (2008) (verbal fluency test), in schizophrenic patients at the beginning of the disease who presented acoustico-verbal hallucination.
The evaluation of the impact of passing the EASE scale on therapeutic alliance, measured, in addition to the clinic evaluation, instrumentally by comparing the Haq-IIp scale before and after EASE, shows an average improvement of 23,94 %, improvement found in 100 %of the cases. Larger studies seem necessary to confirm or invalidate these results.
