Efficacité des anti VEGF dans le traitement de l’œdème maculaire diabétique : une étude de vraie vie

Abstract : Purpose: To evaluate the efficacy and safety or ranibizumab in patients with diabetic macular edema (DME) in population of Afro-Caribbean people. Methods: It was a retrospective analysis of patients with vision loss due to DME and treated with ranibizumab. All patients received a loading dose of 3 monthly injections followed by a treatment according to a PRN (Pro Renata) protocol. The primary study endpoint was the best corrected visual acuity (BCVA) at 12 months. Secondary endpoints were the change in central retinal thickness (CRT) and the number of intravitreal injections(IVI) at 12 months. Results:31 eyes of 24 patients were included. BCVA changed from 59.7 l +/- 14,5 letters (20/63) at baseline to 66,9 +/- 10,9 letters (20/50) at 12 months (p=0,0003; mean gain: +7,3 letters). CRT increased from 441.6 +/- 128.6 µm at baseline to 342.3 +/- 113.9 µm at 12 months (p=0.003). Patients with a baseline VA > 40 letters had a higher final VA of 69 +/- 9 letters (20/40) versus 58 +/- 15 letters (20/63) for patients with a baseline VA < 40 letters (p=0.04). A mean number of 4.4 +/- 1.6 (3-9) IVI were administered. Conclusion: This study conducted in a clinical setting confirms the results of previous randomized trials. The final BCVA was mainly influenced by the baseline BCVA, which supports the utility of early DME treatment before patients experience a severe vision loss.
