Prévalence du surpoids, de l'obésité infantile et du rebond d'adiposité précoce autour de l'étang de Berre. Dépistage et prise en charge en médecine générale

Abstract : Introduction: The fight against obesity is a major public health stake in France. Obesity can sometimes start as early as childhood. As seen previously, in 2003, where among children aged 4, the incidence of overweight reached 8.2% and that of obesity 2.1% in the PACA region. Implementing the National Health and Nutrition Plan in 2001 may have helped stabilize the prevalence of childhood obesity. Purpose of the study: The main objective was to prove that the prevalence of childhood overweight and obesity had stabilized. We were also interested in identifying the incidence of children who were underweight. A secondary objective was to collect data on the prevalence of early adiposity rebound on this population. This study was supplemented by conducting a survey among general practitioners in the territory on the screening and medical care of overweight in children. Material and methods: With the help of maternal and child protection services, we carried out a descriptive quantitative study that included 3489 subjects aged 3.5 to 4.5 years, schooled around the pond of Berre, during the school year 2017- 2018. Definitions of overweight, obesity and underweight used in this study have been outlined by the International Obesity Task Force (IOTF) curves. Results: In our population, obesity affected 2.8% of the population, and 8.7% were overweight. We found that there was a higher risk of overweight among girls. The prevalence of obesity was higher in the cities that were considered to be poorer. The prevalence of underweight reached 16.7%, according to IOTF references (<18.5). The prevalence of early adiposity rebound was 24.2%. Discussion: We found that the rates were comparable to those of 2003, and our results confirm a stabilization of the prevalence of overweight among children since the 2000s. We do not have enough data on underweight and early adiposity rebound to conclude on their evolution over time. We note that one in six children is of normal build, but has an early adiposity rebound, requiring screening and medical care.
