Étude prospective randomisée de l'usure d'un polyéthylène hautement réticulé dopé à la vitamine E versus usure d'un polyéthylène hautement réticulé dans la prothèse totale de hanche à double mobilité par analyse stéréo-radiographique à 3 ans de recul

Abstract : Introduction: the wear of polyethylene (PE) is the main cause of mechanical failures by aseptic loosening of metal-polyethylene couples in hip arthroplasty. The vitamin E doping of a polyethylene would bring a long-term oxidative stability allowing a reduction of the wear. Radio Stereometry (RSA) is an accurate method of measuring PE wear. The aim of this study is to compare the creep and wear of two polyethylenes in a dual mobility prosthesis, E1 (VEPE) and Arcom (UHMWPE) by RSA analysis at 3 years. Material and methods: in this prospective, randomized, CPP-approved study, 65 patients were randomized into two comparable groups: Arcom group receiving UHMWPE (n = 33) and E1 group receiving Vitamin E-enriched UHMWPE (n = 33). = 32). An RSA reference snapshot was made at J7. They were evaluated clinically, radiologically and by RSA at 3 months, 1 year, 2 years and 3 years. Head penetration in PE, a reflection of creep and PE wear, was measured by the distance between the centers of the femoral head and PE by RSA. Results: at 3 months, the RSA measurements found an average PE penetration of 0.143 +/- 0.0038mm in the Arcom group, and 0.163 +/- 0.0027mm in the E1 group (p = 0.117). At 3 years, the penetration of the femoral head was 0.218 +/-0.063mm in the Arcom group and 0.218 +/- 0.033mm in the E1 group with a non-significant difference (p = 0.320). The clinical results are comparable in both groups. Conclusion : vitamin E doping did not influence creep and 3-year wear of UHMWPE in our study.
