Traumatisme crânien : la mise en place des évaluations écologiques en ergothérapie

Abstract : Introduction. – This study focuses on the care given to people with moderate or severe Traumatic Brain Injury (TBI). TBI generates many functional consequences. Through care, the occupational therapist aims at enabling the patient to be more autonomous in a significant environment. To do this, they use ecological assessments. This work is an introduction to research examined occupational therapists' practices concerning the objectives, the setting up and the context of the assessment of these ecological situations. Materials and methods. – A quantitative study was conducted using questionnaires derived from the Canadian Occupational Performance Model, the therapeutic benefit of activity, and a patient-centered approach. This method allows to identify factors that differentiate practices within a sample of occupational therapists. Results. – 38 responses were received out of which 27 came from occupational therapists in rehabilitation wards (inpatient or outpatient) and 11 from medicalsocial services. The professionals take into account the patient as a whole. However, the results showed that occupational therapists practicing in medico-social services take the environment more into account and manage to implement their assessments in more meaningful environments for the patient. However, there was a major obstacle for all the occupational therapists: the lack of time for the implementation of these evaluations. Conclusion. – Ecological assessments seem to stimulate the patient’s commitment and adhesion to their occupational therapy treatment. Occupational therapists’ practices are varied and depend on the location of their professional practice. A quantitative study of patients would be relevant to analyse the interest of ecological assessments from their perspective. It could be cross-checked with the representations that occupational therapists have of their own practices.
