Suivi des patients artéritiques pris en charge par cathéters péri nerveux à domicile pour douleur chronique rebelle dans le sud de la Réunion : étude observationnelle rétrospective de 26 cas

Abstract : Introduction: the peripheral nerve catheterism allows us to relieve the patient from intensive pain like arteritic pain often uncontrolled by usuals drugs. The goal of this study is to describe the evolution of patients suffering from advanced-stage peripheral arterial obstructive disease (PAOD) sent home as outpatients with peripheral nerve catheters on the Reunion island. Méthodology and population: retrospective, descriptive and monocentric study. Sample patients from the Hospital center in the south of Reunion island according to CCAM AHLB009 codage “Continuous anesthetic nerve block”, between December 2012 and March 2017 and thereafter an analysis of medical and paramedical files from the hospital and Home Health agencies. Included patients were suffering from chronic arteritic pain and sent home with peripheral nerve catheters. The latter were put in place by the hospital’s anesthetists according to the protocol designed to this effect in the service. The main assessment criterion was the occurance of secondary complications. Results : 26 patients were included. The average age was 69.9. 76.9% among them were diabetic. The average ratio catheter for patient was 2 for 1. The average length of treatment was 46.29 days. No severe complications occurred. Equipment dysfunction affected 57.7% of patients, suspected infection 34.62% and system inefficiency 23.08%. In 34.62% of the cases, the treatment was brought to a premature end. Conclusion : dealing with chronic pain within ischemic patients bears no major risk and is compatible with outpatient services.
