Impact du jeûne sur le flux de patients aux urgences ainsi que sur les pathologies diagnostiquées

Abstract : Introduction: Ramadan is a fast practiced in the Muslim religion, the second most represented religion in France. This fasting is responsible for a change in eating habits and an inversion of the nycthemeral rhythm, the impact of which has rarely been studied in France. Objectives: To evaluate the impact of Ramadan on the adult emergency departments of the Assistance Publique des Hôpitaux de Marseille (AP-HM) and on pathologies related to fasting. Methods: Epidemiological, observational, etiological, retrospective study from 2010 to 2017 in the emergency departments of the AP-HM, including all visits from June 1 to September 30 of each year. The number of emergency departments admissions per day, admission time and fasting diagnosis were compared according to the non-Ramadan/Ramadan period with univariate analysis (Student). In multivariate analysis, the results were adjusted for factors statistically associated with passages (weather data and year of admission) and diagnoses (ambient temperature). Results: 343880 passages have been included. Regardless of climatic factors and year of admission, the number of visits to emergency services per day was -2% in Ramadan (p<0.001), but the flow of visits decreased by -2.3% from midday to 3 p.m (p=0.049), -2.5% from 3 p.m to 6 p.m (p=0.033), -6.5% from 6 p.m to 9 p.m (p<0.001) and -5.3% from 9 p.m to midnight (p<0.001) then increased by +7.1% from midnight to 3 a.m (p<0.001) and +5.8% from 3 a.m to 6 a.m (p=0.017) during Ramadan. Fasting related pathologies were not more frequent during Ramadan (p=0.835). Discussion: While the number of daily visits decreased in Ramadan, there was a change in the hourly flow of emergency departments visits with an increase in deep night time. These results are in line with the data in the literature. However, the clinical impact appears to be minor, representing about +1 admission/2H at deep night.
