Implication des médecins généralistes dans la prise en charge des patients traités par chimiothérapie pour un cancer à l’Hôpital d’Instruction des Armées Bégin

Abstract : Introduction: patients undergoing antineoplastic chemoterapy endure many symptoms and increased dependence on the health system. This wok aims to identify ways to increase the involvement of general practitioners (GPs) in the management of the population under care at Bégin Army Teaching Hospital (ATH). Method: a survey encouraging Free expression was conducted among patients undergoing chemotherapy at Begin ATH, and followed by semi-structured telephone interviews with their general practitioners. Results were described and qualitatively analyzed. Results: between 15/05/2017 and 02/06/2017, 84 self-questionnaires were given to 62 patients. Subsequently, 27 interviews were conducted with their general practitioners. The most common symptoms were fatigue, digestive disorders, and pain. During the study, 15 non-scheduled care cases were identified. Patients appreciated listening and moral support from their GPs. Only 2/3 of GPs knew their patients were receiving antineoplastic chemotherapy at Bégin ATH. Discussion: among the medical problems causing unscheduled recourse to the health care system in patients treated by antineoplastic chemotherapy at Bégin ATH, a majority could be treated by Gps in an ambulatory setting. The patients' lack of confidence in the specific skills of their GPs and the frequent disengagement of the GPs after the diagnostic phases and the initial orientation of the patient are obstacles to the involvement of GPs during the treatment phase. Good communication between oncologists and general practitioners is a prerequisite for optimal involvement of the latter, as well as patient education and GPs training.
