Le suivi et prise en charge des patients asthmatiques : les obstacles à la bonne application des recommandations : étude qualitative par entretiens semi-dirigés auprès de médecins généralistes des Côtes-d'Armor

Abstract : Introduction: asthma is a disease whose prevalence increases over the years, leading to an additional cost for society, directly related to its good monitoring. Unfortunately, according to several studies, this one is often incomplete. The objective was to highlight these obstacles preventing the good practices of follow-up of an asthmatic patient by a general practitioner Methods: this thesis is a qualitative study based on grounded theorization conducted with general practitioners of Côtes-d'Armor in France by semi-directed interviews Results: the saturation of the data was obtained after 10 interviews with physicians with different profiles. Among the many obstacles, the most important were the lack of time, present at all stages of the follow-up, the lack of therapeutic education and the difficulty of patient acceptance. Asthma was often trivialized by patients and doctors and was frequently a secondary reason for consultation, which was discussed quickly at the end of the consultation. A lack of training was evoked with old decision-making schemes and too many drug specialties. Air pollution was little mentioned, due to the rural and semi-rural patient. Discussion: medication techniques were often misunderstood by their patients and many relied on pharmacists for teaching. A bad initial education complicated the continuation of all the care. In none of the physicians, written action plans were used in adults. Outpatient therapeutic education services were either absent from the region or unknown to doctors.
