Conciliation médicamenteuse et optimisation thérapeutique du SSR1 de l’hôpital Xavier Arnozan (CHU Bordeaux) : évaluation de la satisfaction des médecins généralistes

Abstract : Introduction: facing population aging, prevention of iatrogenic medication and improvement of prescription medication in elderly have become a major public health issue. Medication reconciliation (MR) and treatment optimization (TO) in the geriatric rehabilitation care unit (SSR1), based on a medical-pharmaceutical collaboration, try to answer this issue. MR prevents and intercepts inappropriate drug prescription thanks to a multi-professions sharing of information. TO is elaborated with potentially inappropriate drug lists and complementarity between geriatric physicians and clinical pharmacists. The main objective of our study is to assess general practitioner’s (GP) global satisfaction of MR and TO in geriatric SSR1 ward of Xavier Arnozan’s hospital. Material and methods: transversal and quantitative satisfaction study, based on anonimized telephone interviews, conducted using a questionnaire, of GPs whose a patient benefited from MR and TO in SSR1.Results: 53 GP were included between 01/12/2017 and 29/06/2018, thanks to a high answering rate (82.8%). 45.3% of GPs were very satisfied and 54.7% satisfied. No GP was either poorly satisfied or unsatisfied. The MR and TO stimulated GPs’s great interest for their liberal practice, including the peer reviews by geriatric experts, consultation time optimization and medical-pharmaceutical interdisciplinarity. However, GPs noted their lack of implication. Conclusion: our study allowed to highlight GP’s satisfaction about MR and TO in geriatric rehabilitation service SSR1. The prospect of extending such a collaboration to ambulatory was considered too time-consuming. Prospects of bypassing organizational barriers have therefore been mentioned, including the use of information and communication technologies.
