Prise en charge des principales vulvovaginites en médecine générale‎ : étude quantitative réalisée auprès des médecins généralistes picards par auto-questionnaire

Abstract : Objective: Vulvovaginal pathologies are the most common reasons for gynecological consultation in general practice. The aim of this study was to describe the management of the main vulvovaginitis by general practitioners in Picardie. Methods: A descriptive quantitative study was conducted using a questionnaire sent to 500 general practitioners. It aimed to analyze their diagnostic, educational and therapeutic attitudes for vulvovaginal candidiasis, trichomonas infections and vaginosis. Results: 133 usable questionnaires were received. Vulvovaginal pathologies were managed more out of obligation than interest for 60% of the physicians. The clinical examination was not systematic for a quarter of the respondents. The vagina swab was erroneously performed for vaginosis in 51.1% of cases. The prevention of sexually transmitted infections was tackled in 45.5% of cases for candidiasis, 84.2% of cases for trichomonas infection and 51.9% of cases for vaginosis. Empiric treatments were most often prescribed as first-line treatment, often involving several therapeutic classes. The management of the pathologies was overall satisfactory, with 93.2% of the physicians prescribing topical antifungal agents for candidiasis, 89.5% using antibiotics for trichomonas infection, 72.2% treating vaginosis with antibiotics and 54.1% associating them with probiotics. Conclusion: The practices are not homogeneous in terms of diagnosis or therapy. However, the educational approach shows that the care of the patient remains global.
