Prise en charge chirurgicale de l'infertilité tubo-péritonéale au CHU de Caen entre 2005 et 2014

Abstract : Introduction: this study aims to investigate the surgical management of tubal infertility in our center. Materials and methods: we retrospectively reviewed all the women who underwent a surgical treatment for tubo-peritoneal infertility in the university’s hospital of Caen between 2005 and 2014. We collect pre and postoperative data. The primary endpoint was the live birth rate. Results: seventy-nine patients were included. Live birth rate after tubal surgery for infertility was 54%.44.4% were spontaneous pregnancies, 9.3% resulted from intra-uterine insemination, 46.3% occurred after IVF. Spontaneous pregnancies occurred in a median time of 5 months after surgery. The rate of surgical complication was 2.5% and no major complication happened. The surgery failure rate was 5%. The ectopic pregnancy rate was 5%. Discussion: tubo peritoneal surgery, alone or combined with assisted reproductive technology, is an effective procedure for the management of female infertility. The tubal damage evaluation and the nature of the surgery procedure must help to determine the spontaneous fertility prognosis. The delay after tuboperitoneal surgery within a couple must be provided guidance into IVF should include the tubal prognosis and the patient age. Thus, a 12month delay must be restricted to young women with light or mild tubal damage and a promising tubal surgery.
