Influence du diamètre de l’implant phonatoire sur la qualité de vie et de voix trachéo-oesophagienne

Abstract : Objective : After total laryngectomy, in a context of tracheoesophageal vocal rehabilitation, the initial placement of a decreased diameter prosthesis could prevent the widening of the tracheoesophageal fistula, which is responsible for periprosthetic leakages. The aim of this study is to ckeck that this decreased diameter has no significant influence on quality of life and quality of the tracheoesophageal voice.
Methods: 7 patients with a 16Fr prosthesis and 10 patients with a 22,5Fr prosthesis were included. The perceptual assessment was performed with the (i)INFVo scale, the quality of life self-assessment was measured by the Voice Handicap Index (VHI), and the aerodynamic measurements of the 4 Maximum Phonatory Time (MPT), number of syllables per expiration, subneoglottal pressure, and oral airflow were performed.
Results: No significant differences between the groups were found for VHI (total score: p = 0,254), MPT (p=0,7696), number of syllables per expiration (p = 0,1874), subneoglottic pressure (p=0,0619), oral airflow (p = 0,083) and neoglottic resistance calculated from these two values (p = 0,6892). For the (i)INFVo, significantly higher scores for the 22,5Fr group are found for general impression, presence of noises, and fluence.
Conclusion: This study allows us to conclude that the placement of a decreased diameter prosthesis does not affect the quality of life or the aerodynamic parameters of the tracheoesophageal voice, compared to a speaker with a standard size prosthesis.
