General practioner’s place in the long term care of patients whith traumatic brain injury admitted to intensive care unit: a study on the NEURODATA’s cohort during the year following their hospitalisation

Résumé

Introduction: traumatic brain injury is a common and serious pathology. There is no recommendation from scientists after 24 hours of management care. Our study aims to observe the medical monitoring of the NEURODATA cohort by their general practitioner (GP) and in second time by the university of Caen in the year after their hospitalization in intensive care unit. Material and method: the data were collected after a call of the patient’s GP and on consultation of their medical file at the university hospital of Caen. Patients were grouped into good and poor prognosis according to the Glasgow Outcome Scale Extended. Results: 99 patients were included, 39 are dead, 47 had a prognostic evaluation at 1 year, for 31 of 47 patients (66%), the GP were called: 26 patients had a good prognosis and 5 a bad one. Patients with good prognosis were followed more by GP (58%) than patients with poor prognosis (p = 0.007). The medical monitoring at the university center of Caen at 3 months was made for 61% of patients: those with a good prognosis were more followed than those with poor prognosis. They were also more often referred to specialists by their GP. Conclusion: patients with good prognosis at 1 year are followed by GP, contrary to poor prognosis most often institutionalized with specialized follow-up. The GP’s target is the population with a good prognosis at 1 year with mission to prevent secondary complications of patients with TBI hospitalized in intensive care unit. Multidisciplinary consultations should be established to assist GP in their work.
Introduction : le traumatisme crânien est une pathologie fréquente et grave. Or, il n’existe pas de recommandation des sociétés savantes après 24 heures de prise en charge. Notre étude a pour but d’observer le suivi médical de la cohorte NEURODATA par leur médecin traitant (MT) et secondairement par le CHU de Caen, dans l’année qui a suivi leur hospitalisation en réanimation. Matériel et méthode : les données ont été recueillies après appel du MT des patients et sur consultation de leur dossier médical au CHU de Caen. Les patients étaient groupés en bon et mauvais pronostic selon le Glasgow Outcome Scale Extended. Résultats : sur 99 patients inclus, 39 sont décédés, 47 ont eu une évaluation pronostique à 1 an, pour 31 des 47 patients (66%), les MT ont été contactés : 26 patients avaient un bon pronostic et 5 un mauvais. Les patients de bon pronostic ont été plus suivis par les MT (58%) que ceux de mauvais pronostics (p=0.007). Le suivi au CHU de Caen à 3 mois a été réalisé chez 61% des patients : ceux de bon pronostic étaient plus suivis que ceux de mauvais pronostics. Ils étaient également plus souvent adressés à des spécialistes par leur MT. Conclusion : les patients de bon pronostic à 1 an sont suivis par les médecins généralistes, contrairement à ceux de mauvais pronostics le plus souvent institutionnalisés avec un suivi spécialisé. Le MT cible une population de bon pronostic à 1 an avec pour mission de prévenir les complications secondaires des patients traumatisés crâniens passés en réanimation. Des consultations multidisciplinaires pourraient être instaurées pour aider les MT dans leur travail.
