Consultation 2.0. Approche de la perception de la diffusion des informations médiatiques en matière de santé dans la pratique des médecins généralistes. Étude qualitative par Focus Group auprès de médecins généralistes des Bouches-du-Rhône et du Vaucluse

Abstract : Introduction: Media information's broadcast about health is a growing phenomenon that, although old (traditional media) benefited from the internet revolution (e-Health 2.0). As few studies exist, the objective was to analyze their real-life experience of the phenomenon in the practice of general practitioners, and secondly to know if it resulted in modifications in their exercise and relationship with the patient. Method: It was a qualitative study with data collection through focus group and individual interviews, then analysis with Grounded Theory. Were included liberal general practitioners or interns of SASPAS in Bouches-du-Rhône and Vaucluse, recruited on a voluntary basis by multiple requests (mail, e-mails, SMS, word-of-mouth). Were excluded those refusing to participate or not having answered. Results: Between November 2017 and July 2018, 4 focus groups and 3 individual interviews (25 participants) have been conducted until final saturation. Their perception was ambivalent (positive, negative or neutral) according to several factors. Only the experience of the Levothyrox ®case was unanimously negative (defect of information from the sanitary authorities hence an impossibility of adaptation). About the relationship patient-doctor, changes were noticed in the position of both towards the medical knowledge. The role of the patient, better informed, was valued (proactive way of information retrieval, autonomy and accountability) and the doctor’s one was « desecrated », desecration finally identified as beneficial, allowing an enriching partnership in which the doctor always has his place in his office for human reinsurance. To adapt itself, by preserving the bases of the profession, a new educational function was accepted : being the middleman patient-media. Supported by the State and others health-care professionals, engaging in the media information seems important. Conclusion: The general practitioner must be supported in upheavals stemming from media. The future of health (Telemedicine and other new technologies) aiming at desacralizing other practitioner's skills, studies should still give the floor to him.
