Place des médecins généralistes dans le dépistage des lésions cutanées malignes dans la région Provence Alpes Côte d'Azur

Abstract : Introduction: Skin cancers are the most common cancers in France and represent a global public health problem. For 40 years, their frequency has experienced major growth; their prognosis is strongly correlated with the precocity of their care. Early detection of malignant skin lesions increases overall patient survival and reduces public health costs. Our study describes the practice of general practitioners in the Provence Alpes Côte d'Azur region when they are confronted with a malignant cutaneous lesion and observes their level of involvement in this screening. Method: We conducted a practice survey by distributing a questionnaire to general practitioners in the PACA region to assess their screening habits and their attitudes to a series of photos of dermatological lesions. 103 doctors were included in our study. Results: We observe that in 96.26% of cases the attitude of the general practitioner is adapted to a malignant cutaneous lesion. However the benign lesions were addressed to a dermatologist in 65.8% of cases, resulting in an over-sollicitation of the specialist. We found that dermatological screening is practiced with only 17.5% of the doctors in the study who perform specific dermatological screening tests. The main obstacles are the lack of time in consultation, the fear of reassuring and the feeling of lack of knowledge in dermatology. Conclusion: Our study highlights the ability of general practitioners to participate effectively in screening for skin cancers. However, this screening is under practiced and has many challenges. It appears essential to look for ways to improve such as the development of telemedicine, the reimbursement of oncological screening consultations or the improvement of training in dermatology of general practitioners.
